You will come across the Mitragyna speciosa tree, called kratom, mainly in Southeast Asia, especially in Thailand, Malaysia and Indonesia. Asians attribute healing effects to it and it has become an integral part of their traditional medicine. Kratom leaves are chewed directly or crushed and made into a tea. In the Western world, we are used to taking kratom in the form of powdered extract, pressed tablets, powder in gel capsules or as a tea. Users perceive it as a quick aid for pain relief, and some even combine it with other substances for more complex effects.

How does kratom work in the body? Mainly alkaloids

Kratom owes its effects to two alkaloids: mitragynine and 7-hydroxymitragynine. These substances bind to opioid receptors in the body and probably cause an effect similar to morphine, but 3x to 4x weaker. You would need to take an estimated 15 to 20 mg of mitragynine to get the desired effect, while 4 to 5 mg of morphine. Thus, at low doses, kratom behaves as a mild stimulant, while at higher doses, it approaches the effects of opioids.

For idea: A 70 kg man would need to take 60 to 80 mg of mitragynine for moderate pain relief.

How long does mitragynine take to start working?

The effect of this alkaloid begins after 5 to 30 minutes of use and lasts approximately 1,5 to 6 hours.

What did kratom research show in mice?

Research in mice has revealed that mitragynine itself is not as potent as previously thought. Its effects are due to the metabolite 7-hydroxymitragynine. However, the amount of 7-hydroxymitragynine in the body varies from person to person and depends on their metabolism, mainly on the activity of the CYP3A4 enzyme. This probably leads to a so-called "ceiling effect". After a certain dose of mitragynine, the concentration of 7-hydroxymitragynine in the body (in the brain) does not increase further.

What does that mean for you?

Mitragynine probably won't relieve severe pain in most people, but it works for mild to moderate pain. Compared to morphine, this alkaloid has less side effects. However, users report that higher doses help them manage pain and ease opioid withdrawal symptoms.

How long does kratom stay in the body?

This plant raises many questions not only among researchers, but also among users themselves. One of them is how long it lasts. The experience of farmers in Southeast Asia suggests that it can ward off fatigue and keep them productive for several hours.

However, we have no official information on how long kratom lasts in the body. We can base it on pharmacological studies that investigated the properties of mitragynine and its half-life, i.e. after how many hours 50% of the original dose is found in the body. According to these studies, the half-life of mitragynine ranges from approximately 3,85 to 9,43 hours. Some sources put the half-life as long as 23,34 hours.

Attention, sensitive people may observe side effects

Kratom can cause a number of side effects in sensitive people such as dry mouth, nausea, rapid heartbeat, hallucinations or psychosis. Unlike morphine, kratom is less likely to cause respiratory depression. Even so, there is a risk of overdose. Fortunately, there is no documented case of fatal poisoning with pure mitragynine.
